IDPH Posts Nursing Home Violations for First Quarter of 2025

[Of rhe four facilities listed below…cited with type “AA” violations of the Nursing Home Care Act during the first quarter of 2025, the most severe type of violation, [one was from Huntley].  An “AA” violation is cited when there is a condition or occurrence at the facility that proximately caused a resident’s death. 

The resident had trouble swallowing.

Among the notes, all of which can be read by clicking on the link above, are the following:

“On 10/15/24 at 11:07AM, V5 (LPN) stated, ‘If a physician or nurse practitioner orders a resident to have 1:1 supervision, then that means staff are to be sitting next to the resident during mealtimes supervising them. If staff are not sitting with the resident and providing them cues or assistance, they could choke.’

“‘I was working the day that (R2) had his choking incident. I was outside of the dining room passing medications and when I looked in, (R2) was blue. I called for help and (V8) ran in to help me and performed the Heimlich. We got (R2) to his room as fast as we could to finish caring for him while 911 was on their way. From what I remember, there were no staff members feeding (R2) otherwise they would
have yelled for help when he started choking.'”

“R2’s death certificate dated 10/1/24 showed, ‘Cause of death: complications from aspiration pneumonia, choking on food.'”
